 Applications

We will start taking applications on Monday, Sept, 20th -  Dec.13th

 Must Provide:

· Proof of income (last 4 pay stubs, OWF/Disability/SSI/Social Security)

· Proof of residency (recent utility bill)

· Social Security Numbers (for everyone in the household)

· Picture ID

· Must Live in Sylvania School District

Dear Friends:

The holiday seasons are fast approaching and Sylvania Area Family Services is starting preparations for our holiday baskets.

Last year, with the help of many individuals and organizations, SAFS was able to provide approximately 450 holiday baskets to families in need.  

Once again, we are looking for individuals or groups who are willing to “adopt” Sylvania area families for Thanksgiving and/or Christmas.  By adopting a family, you can assist in making a brighter holiday season for these friends and neighbors in their time of need.      

Holiday Baskets are as follows:

Thanksgiving

• donors may choose to provide meat, groceries, a gift certificate or any combination of these items

Christmas

• donors have the same basket choices with the additional option of providing gifts for their family

We will provide family profiles for all donors who contact us no later than Friday, November 12^th for Thanksgiving and Friday, December 10^th for Christmas.  We will gladly accept donors after these dates but we may be unable to provide profiles.
