To Be Eligible for Holiday Assistance, You Must:

• Be a resident of the Sylvania School District.

• Have at least one minor child living in your home.

• Meet the income eligibility requirements listed below.

Important Information:

• You may only submit one application per household. Applications including individuals not living in your household or duplicate submissions will be deleted.

•  Please note that if you have already signed up for Holiday Assistance with another organization, you will not be eligible to sign up with Sylvania Area Family Services.

• This application covers both the Thanksgiving and Christmas holiday assistance.

Required Documentation to Upload:

• All applicants must provide proof of residency within the Sylvania School District.

• You must be receiving assistance from JFS or be enrolled in the Free and Reduced Lunch Program within the Sylvania School District.

• Upload a current copy of your assistance benefits (NOA letter) and/or Free and Reduced Lunch Program determination letter from Sylvania Schools, and a current utility bill within 30 days of application.

Verification and Pickup:

• All information is subject to verification.

• If your application is approved, you will receive an email with your pickup date and time.

• A valid ID must be presented at the time of pickup.
